Text Box: State law says children should not be allowed to enter school without the proper immunizations. New students entering Kindergarten or transferring into our system must present up-to-date immunization record before attending school. Call your healthcare provider for a copy of your child’s immunization record.

4 doses of OPV or IPV.  If the third dose was given on or after the 4th birthday, the 4th dose is not required.  However, if  a combination of IPV/OPV  is used then all 4 doses are needed regardless of age.

3 doses for Kindergarten entrance, 7th grade entrance and for transfers grades K thru 11th.

2 doses .  Second dose should be at least 30 days after the first dose.

4 or more doses (One which was given after their 4th birthday.

Polio or (OPV / IPV)   

Immunizations

DPT  or (DTaP - DT - Td)  

1 dose for Kindergarten entrance or history of disease provided by parent or physician

Hepatitis B or (HBV)      

Varicella (Chickenpox
